North Staffordshire CCG and Stoke on Trent CCG are looking for expressions of interest for the tender of the memory support service. This service will provide support to patients and carers who are awaiting or have received a dementia diagnosis through the provision of a named care co-ordinator following the dementia well pathway. The service will need to be available/accessible to all residents of the 2 CCG's with innovative and individual support provided to both the patient and the carer. The service will help to prevent unnecessary admissions to hospital and assist with timely discharge when a patient has been admitted.
