The subject of the contract is the provision by the Contractor of successive delivery of drugs covered by the drug program for the treatment of moderate and severe plaque psoriasis and atopic dermatitis AD in adults who are eligible for general treatment and the delivery of various medicinal products for the 105th Kresowy Military Hospital with the SP ZOZ Outpatient Clinic in Żary

Delivery of the biological drug Dupilumab covered by the drug program for the treatment of patients with severe atopic dermatitis AD. The contractor's duties will include successive delivery of the biological drug Dupilumab covered by the drug program for the treatment of patients with severe atopic dermatitis AD in the quantities and assortment specified in the assortment and price form, constituting Annex 9 to the SWZ.

The contractor's duties will include successive delivery of the biological drug Ixekizumab covered by the drug program for the treatment of moderate and severe plaque psoriasis in adults who qualify for general treatment in the quantities and assortment specified in the assortment and price form, constituting Annex 10 to the SWZ.

The contractor's duties will include the delivery of the biological drug Risankizumab covered by the drug program for the treatment of moderate and severe plaque psoriasis in adults who are eligible for general treatment in the quantities and assortment specified in the assortment and price form, constituting Annex 11 to the SWZ

The contractor's obligations will include the delivery of the biological drug Guselkumab covered by the drug program for the treatment of moderate and severe plaque psoriasis in adults who are eligible for general treatment in the quantities and assortment specified in the assortment and price form, constituting Annex 12 to the SWZ.

The contractor's duties will include the delivery of the biological drug Ustekinumabum covered by the drug program for the treatment of moderate and severe plaque psoriasis in adults who are eligible for general treatment in the quantities and assortment specified in the assortment and price form, constituting Annex 13 to the SWZ.
