1. The subject of the contract is the supply of medical equipment specified in the Parts for the Gynecology and Obstetrics Clinic of the Fryderyk Chopin University Clinical Hospital in Rzeszów.

A detailed description of the subject of the contract, the quantity of deliveries and the requirements are included in the Description of the subject of the contract – a list of parameters required for Lots No. 1, 2 and from No. 4 to No. 8, in the Description of the subject of the contract – a list of parameters required and evaluated for Part No. 3 (Annexes No. 1 to the SWZ) and Price and offer forms for Parts No. 1 to No. 8 (Annexes No. 2 to the SWZ), which are an integral part of the SWZ.

3. The scope of the contract includes the delivery of the objects of the order in the quantities specified in the CMS.

4. The Contracting Authority shall not allow equivalent bids to be submitted in Lots No. 1, 2 and No. 4 to No. 8.

5. The Ordering Party allows in Part No. 3 solutions equivalent to those described in the description of the subject of the contract.

3. The scope of the order includes the delivery of items of the order in quantities:

a) Part No. 1 - Apparatus for transcutaneous measurement of carbon dioxide tcpCO2 - 1 piece,

described in Appendices No. 1 and 2 to the SWZ.

4. The Contracting Authority shall not allow the submission of equivalent tenders.

5. Delivery of the objects of the order in Part No. 1 – up to 4 weeks from the date of conclusion of the contract to the place indicated by the Ordering Party together with the signing of the delivery and acceptance protocol.

4. The Awarding Entity allows submission of equivalent offers.

5. Delivery of the objects of the order in Part No. 3 – up to 4 weeks from the date of conclusion of the contract to the place indicated by the Ordering Party together with the signing of the delivery and acceptance protocol.
