The subject of the contract is the purchase of 2 type B ambulances with medical equipment and equipment for the Hospital Emergency Department. A detailed description of the subject of the contract is included in Annex No. 1A to the specification constituting an integral part thereof.
a) The Ordering Party requires offering 2 pieces of brand new, without operational mileage, manufactured not earlier than in 2023 type B ambulances with medical equipment and equipment, with technical parameters consistent with the description of the subject of the contract constituting Annex 1A to the SWZ.
b) The subject of the contract must be free from defects, covered by the manufacturer's warranty, meeting the requirements of the applicable Polish Standard PN-EN 1789 and PN-EN 1865 to the extent appropriate to this procedure.
c) The subject of the contract must have a valid approval certificate for the base vehicle and the completed ambulance (base car with medical body). The approval of the offered ambulance must allow registration in Poland.
d) The offered medical equipment must be approved for use on the territory of the Republic of Poland and have current documents confirming marketing authorization in the Republic of Poland in accordance with the provisions of the Act of 07 April 2022 on medical devices and executive acts to the Act.
e) The Contractor shall provide a guarantee for the subject of the contract, the validity of which will start from the date of signing the vehicle acceptance protocol:
1) for the base vehicle (without mileage limit) – for a period of not less than 24 months,
2) for medical buildings and medical equipment – for a period of not less than 24 months,
3) for the perforation of the ambulance body – for a period of not less than 60 months.
f) Together with the delivery of the subject of the contract, the Contractor shall provide the Contracting entity in Polish with all documents necessary for its use, in particular:
1) user manual for the base car, medical body and medical equipment in paper and electronic versions,
2) guarantee document,
3) technical passports completed and signed by the service technician with the date of the next technical inspection (applies to medical equipment),
4) service inspection books,
5) required by applicable regulations: approval certificate for base vehicle and completed ambulance (base car with medical body); vehicle card, other necessary certificates and certificates of compliance with the Polish Standard PN-EN 1789 and PN-EN 1865.
g) On the day of delivery of the subject of the contract, the Contractor is obliged to train medical personnel in the operation, operation and maintenance of the ambulance and medical equipment at the headquarters of the Contracting Entity.
h) The Contractor is obliged to provide a valid certificate of exemption from excise duty of the ordered ambulance before delivery. Within a period of up to 2 months from the delivery of the subject of the contract, the Contractor shall provide the Contracting Authority with a document exempting from excise duty issued by the competent Customs Office, which will contain the necessary identification data of the ambulance.
