The subject of the contract is: Execution of the project documentation for the investment task of the following: “Construction of the road junction at the junction of the DK national road no. 81 with the provincial road DW 944 — Bielska street in Skoczów”, as well as support of the Ordering Party at the stage of the tender for construction works together with providing explanations and answers to tenderers and carrying out copyright supervision during the implementation of works carried out on the basis of the prepared design documentation in the amount of 60 stays.Intended to design the investment includes the construction of a road junction at the site of the existing intersection national road no 81 with provincial road DW 944 — Bielska street in Skoczów, together with the removal of collisions with technical infrastructure not related to the road. The investment will be carried out on the DK 81 section from km 61 +270 to 62 + 420. The object of the contract is: The execution of the project documentation for the investment task of the north: “Construction of the road junction at the junction of the DK national road no. 81 with the provincial road DW 944 — Bielska street in Skoczów”, as well as support of the Ordering Party at the stage of the tender for construction works together with providing explanations and replies to tenderers and carrying out authorial supervision during the implementation of works carried out on the basis of the project documentation produced in the amount of 60 stays. Intended to be designed, the investment includes the construction of a road junction at the site of the existing intersection of national road No 81 with the provincial road DW 944 — Bielska Street in Skoczów, together with the removal of collisions with technical infrastructure not connected with the road. The investment will be made on the section DK 81 from km 61 +270 to 62 + 420.The Ordering Party notes that the range of ±50m from the indicated kilometres should be taken into account at the valuation of the offer.

The requested documentation will include, among others, the following elaborations:software concept, materials for the application for a decision on the environmental conditions of consent for the implementation of the project, construction project with materials for the application for the decision of ZRID, executive project tender materials. 2. The scope of the object of the contract in particular includes: 1) the execution, in accordance with the provisions of the SWZ and the Designed Contractual Provisions (PPU), of the design documentation set out in Annex 2 to the SWZ and Annex 1 to the PPU - Price Form/Stage Table contract [TEU], together with all necessary elaborations, materials, arrangements and activities necessary for the execution of this documentation and preparation of the task, and obtaining the necessary administrative decisions (Phase I, II, III, IV); 2) to develop responses to Contractors' inquiries concerning the design documentation during the procedure for the award of a public works contract and/or the preparation of changes to the design documentation (Phase V); 3) the exercise of copyright supervision of the undertaking, during construction works carried out on the basis of documentation design developed by the Contractor, and during the period of warranty, in accordance with the provisions of the PPU, hereinafter referred to as the Copyright Supervisor (Stage VI). 3. The Ordering Party, GDDKiA (in the field of national roads) and the ZDW (in the field of provincial road) reserves the right to comment at any stage of the development 4. The Contractor will perform a 3D visualization (in the form that allows you to run the presentation on windows - on a cd/dvd board or other widely available data media) imaging the model of the selected design concept, in the form of graphics in at least 3 views. Visualization is intended to depict the building object in general outline (without the requirement to faithfully reproduce the details of the object's surroundings and without the requirement to faithfully recreate the dimensions of the object (visualization for project promotion) .5. The requested design documentation shall be performed in accordance with all requirements under applicable law and

1) by the orders of the General Director of National Roads and Highways and to the extent enabling it to be approved by the GDDKiA Branch in Katowice (Investment Undertaking Assessment Team) and the GDDKiA Headquarters in Warsaw (Commission for Evaluation of Investment Undertakings),

2) the guidelines of the Board of Provincial Roads in Katowice (ZDW) to the extent enabling the approval of it by the ZDW. 6. GDDKiA in the field of national road and the ZDW are entitled to: 1) review and comment on the documentation at the design stage, 2) request to convene and carry out the Technical Councils within the deadlines indicated by themselves, 3) request for amendments, in particular aimed at improving the sustainability of the undertaking or technical parameters of the proposed solutions,4) the possibility of refusal to agree, in particular in cases of incompatibility of the documentation with technical conditions or procedures applicable to the road manager.
