1. The subject of the contract is the supply of cannulas for intravenous infusions for the needs of the entire hospital for the UCC in the assortment, estimated quantity and requirements specified in Appendix 3 to the Terms of Reference.

2. The Contracting Authority does not allow the submission of partial offers due to one subject of the contract, which cannot be divided.

3. The Ordering Party requires that the offered assortment meets the requirements for medical devices contained in the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974) and in executive acts issued on the basis of this Act.

4. Medical devices must meet the requirements set out in Annex 3 to the SWZ. Failure to meet at least one of the requirements set out in Annex 3 to the SWZ will result in rejection of the offer.

5. Delivery of medical devices of this order will be carried out on the basis of partial orders. The maximum delivery date is 6 working days from the date of placing the order by the Ordering Party.

6. The Contractor shall be obliged to deliver the medical devices referred to in paragraph 1 at its own expense to the Medical Supply Department, 17 Smoluchowskiego Street.
