The subject of the order is the delivery to the Health Care Team in Lidzbark Warmiński, assembly, installation and commissioning of a computer tomograph with the adaptation of the rooms on the lower ground floor of the hospital building to a computed tomography laboratory. A detailed description of the subject of the contract for the delivery of a computed tomograph is included in Table No. 1 "Description of the subject of the contract - computer tomograph - requirements" constituting Annex No. 1 to the ToR.

The subject of the contract regarding the adaptation of rooms for the TK workshop is the preparation of design and executive construction documentation in accordance with the Functional Program constituting Annex No. 2 to the ToR and the adaptation of the rooms to the TK workshop. The Contractor will be required to obtain a building permit and occupancy permit for the investment.

The subject of the contract is the delivery to the Health Care Team in Lidzbark Warmiński assembly, installation and commissioning of a computer tomograph with a contrast injector, CD / DVD recorder / printer, ups, operator console, general diagnostic console and other components as well as integration and consolidation of the offered software with medical activity software used by the Employer, training employees in the operation and operation of a CT scanner and other devices. A detailed description of the subject of the contract for the delivery of a CT scanner can be found in the Table titled "Description of the subject of the order - computer tomograph - requirements" constituting Annex 1 to the ToR.

The Part I Contractor will be required to cooperate and cooperate with the Part II Contractor (and with the Employer) including consultations at the place of adaptation work in the area of adaptation of rooms to the CT workshop in terms of meeting the conditions enabling the Part I Contractor to properly assemble, install and launch CT. To this end, the Contractor of Part I will indicate and delegate competent people with specialist experience and knowledge in the field of construction works, assembly, installation and commissioning of CT.

The subject of the contract for the adaptation of rooms on the lower ground floor of the hospital building to a computed tomography workshop is the preparation of design and executive construction documentation in accordance with the Functional Program, the requirements of the computed tomograph manufacturer provided by the Contractor of the first part of the contract, the requirements of applicable legal provisions regarding the subject of the contract and design and construction and adaptation of rooms on the lower ground floor of the hospital building intended for a computed tomography laboratory in accordance with the above-mentioned documentation. The Contractor will be required to obtain a building permit and occupancy permit for the investment.

The functional and utility program is attached as Annex 2 to the ToR. The projection of the rooms for adaptation to the TK studio is annexed to No. 12 to the ToR.

Employer pursuant to art. 29 paragraph 3a of the PPL Act requires that all physical works related to the performance of all works covered by the contract, the performance of which consists of performing work in the manner specified in art. 22 par 1 'of the Labor Code were performed by persons employed by the Contractor or Subcontractor on the basis of an employment contract.

The Contractor or Subcontractor shall employ the above persons for the period from the commencement to the end of the contract period; in the event of termination of employment by one of the parties, before the end of this period, the Contractor / Subcontractor will be obliged to employ another person for this place.
