The subject of the contract is: delivery, installation, commissioning of a medical device - a computer tomograph along with the adaptation of rooms for the Specialist Hospital in Jasło.

As part of the implementation of the subject of the contract, the Contractor is obliged to perform all activities described in detail in the SWZ in this Appendix No. 2 – Description of the Subject of the Contract (hereinafter OPZ), Appendix No. 1 – Offer Form.

The task is co-financed from the COVID-19 Prevention Fund.
