1. The subject of the contract is: purchase and delivery of disposable medical devices for medical entities of the Podkarpackie Voivodeship. The description of the subject of the contract is included in Annex 4 to the SWZ - Offer and price form.
2. The complete offer consists of (obligatory):2.1.The offer prepared in accordance with the template of Annex 1 to the SWZ. Offer form. 2.2. Completed Offer and price form in accordance with the template constituting Annex 4 to the SWZ.
3. The contractor shall submit together with the tender:
3.1. Declaration in the form of an ESD according to Annex 2 to the SWZ;
3.2. Declaration on the absence of grounds for exclusion according to Annex 8 to the SWZ.
3.3. Declaration according to Annex 7 to the SWZ – if applicable;
3.4. Power of attorney or other document confirming authorization to represent the contractor.
3.5. The means of proof listed in Chapter IV, point 6 of the SWZ .
