The subject of the contract is services in the field of forest management as defined in Article 6 (1) point 1 of the Act of 28 September 1991 on forests (text one: Journal of Laws of 2021 item 1275 as late as “Forests Act”) covering works in the field of forest breeding and protection, fire protection, harvesting and ripping of timber to be carried out in the Kańczuga Forest District

The Contracting Authority provides for the possibility of awarding the contracts referred to in Article 214 (1) (7) of the PZP within 3 years from the date of the award of the basic contract.

The scope in kind of services which are the subject of contracts shall not exceed 50% of the value of the procution.The procedure shall be carried out in accordance with the rules laid down for the so-called 'reverse procedure' referred to in Article 139 (1), (3) to (4) of the PZP

The Contracting Authority shall require employment by the Contractor or subcontractor on the basis of the employment relationship of persons carrying out the execution of the contract involving the acquisition and rip-off of wood raw material, if the execution of such activities involves carrying out the work in the manner laid down in Article 22 § 1 Act of 26 June 1974 - Labor Code (text one: Journal of Laws of 2020 item 1320 as amended).

9.2 In order to meet the conditions of participation in the proceedings, the Contracting Authority shall, before selecting the most favourable offer, call on the Contractor whose offer has been highest-rated to submit within the specified time limit, not less than 10 days, the current current as of the date of submission of the following relevant measures evidentiary:

(a) a list of services performed, and in the case of recurring or continuous benefits, also exercised, during the last 3 years (back from the date on which the deadline for submission of tenders), and if the operating period is shorter, including their gross value, object (type of service performed), dates of performance (dates of daily start and completion of services) and entities to whom services have been performed or are duly performed

If the Contractor relies on experience in the performance of services carried out jointly with other Contractors, the list referred to above concerns the services in which the Contractor directly participated and in the case of recurring or continuous benefits in which the performance of directly participated or participates.

(b) evidence determining whether the Contractor indicated in the list of services to confirm the fulfillment of the condition for participation in the procedure concerning technical or professional capacity in the field of experience have been performed or are performed duly,

The evidence referred to above is references or other documents drawn up by the entity to which the services were performed and in the case of recurring or continuous benefits, and if the Contractor for reasons independent of him is unable to obtain these documents — a declaration by the Contractor; in the case of recurring or continuous benefits, references or other documents proving their proper performance should be issued within the last 3 months (back from the date on which the deadline for submission of tenders expires).

(c) a list of persons addressed by the Contractor for the performance of the public contract, in particular responsible for the provision of services, including information on their professional qualifications, the rights (if required), necessary for the performance of the public contract, and the scope of their activities and information on the basis to dispose of those persons

(d) list of technical equipment available to the Contractor for the purpose of executing a public contract together with information on the basis for disposal of these resources
