1. The subject of the contract is the Delivery of medicinal products for the needs of Clinics / Departments / Outpatient Clinics of the Central Clinical Hospital of the Medical University of Lodz at Pomorska 251, Czechosłowackie 8/10, Sporna 34/50.

2. The order was divided into 34 lots.

3. A detailed description of the subject of the contract is attached as Annex 1 to the SWZ – Assortment and price form.

4. Deadline for standard orders – within max. 48 hours (working days Mon-Fri)

5. Deadline for urgent orders – within max. 24 hours (working days Mon-Fri).

6. The deadline for the completion of orders "To Save Life" – within a maximum of 6 hours (the Ordering Party requires delivery of medicinal products in the To Save Life mode on Saturdays, Sundays and holidays and a direct contact number with the person executing the order from the Contractor's side) – in case of doubts whether a given drug is treated by the Ordering Party as a drug "to save life" (if applicable).
