1. The subject of the order is the delivery of reagents for laboratory tests with the lease of analyzers, implementation and inclusion in laboratory and hospital IT systems.

2. A detailed description of the subject of the order together with the quantities ordered is included in Annex 2 to SWZ (Assortment and Price Form) and Annex 3 to SWZ (Draft Agreement).

3. The conditions for delivery and receipt of the object of the order are set out in the draft contract constituting Annex 3 to the SWZ.

4. The offered assortment should be authorised for marketing and use in the territory of the Republic of Poland and comply with the requirements laid down by the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2021 item 1565, as amended).

The type and quantity of supplies were specified in SWZ.
