The subject of the contract includes the supply of electricity for the own needs of the Góra Commune, its organizational units, company, cultural institutions in the period from 01.01.2024 to 31.12.2024, the forecast amount of consumption is 4 116 778 kWh and concerns approximately 223 PPEE. Statements and documents that the Contractor should submit together with the offer: 1) a completed offer form together with a completed table of forecasted consumption, the model of which is attached to the SWZ. The estimated amount of energy supplied during the contractual period is forecast at approximately 4,116,778 kWh and concerns approximately 223 PPEE. Detailed description in SWZ. Before selecting the most advantageous offer, the contracting authority shall call the contractor whose bid has been rated the highest to be submitted within the prescribed period, not shorter than 5 days, valid as at the date of submission of the statements1) the statement referred to in Article 125(1) of the PPL in the form of the European Single Procurement Document, hereinafter referred to as the "ESPD",2) a statement on the absence of grounds for exclusion in the circumstances indicated in Article 125(1) of the PPL. Article 7(1) of the Act on special solutions for counteracting support for aggression against Ukraine and for the protection of national security and in Regulation 2022/576, listed in the SWZ, the template of which is attached to the SWZ, hereinafter also referred to as the sanction declaration. Subjective means of evidence, which the Contracting Authority will call the Contractor to submit after the deadline for submitting tenders on the principles described in Article 126(1) of the PPL:

1. An extract or information from the National Court Register or from the Central Registration and Information on Economic Activity, within the scope of Article 109 paragraph 1 point 4 of the Act, prepared not earlier than 3 months before its submission, if separate regulations require entry in the register or records, if the Contractor did not indicate the address of a free and publicly available database in the statement under Article 125 paragraph 1 of the PPL, respectively, in which they are available and the Ordering Party can download them himself/ 2. Information from the National Criminal Register within the scope specified in Article 108(1)(1), (2) and (4) of the PPL, issued no earlier than 6 months before its submission. In the case of Contractors who have their registered office or place of residence outside the Republic of Poland, they submit documents on the terms described in the Regulation of the Minister of Development, Labour and Technology of 23 December 2020 (Journal of Laws of 2020, item 2415), taking into account the changes introduced by the REGULATION OF THE MINISTER OF DEVELOPMENT AND TECHNOLOGY of 3 August 2023 amending the Regulation on subjective evidence and other documents or statements, which may be demanded by the contracting authority from the contractor, effective from 22 September 2023.
