Description:

NHS Bedfordshire Clinical Commissioning Group, Bedford Borough Council, and Central Bedfordshire Council have agreed to jointly procure Community Health Services. The Procurement partners have agreed an overarching vision for future Community Health Services, which has been developed with key stakeholders across the two Boroughs. The Commissioners have decided to approach this Community Health Services procurement via the use of three lots. However, Commissioners are clear that their overarching requirement is for a Provider who can offer the best outcomes for each lot, whether that results in one Lead Provider across all 3 lots, or a different Provider for each lot.

The total potential value of the contract for all initial and future services is 443 685 200 GBP exclusive of VAT over the full 7 year term ( 5 yεαrs plus 24 months optional term) across the 3 Lots. However, the initial contract value and budget will be 38 242 600 GBP per annum, this is inclusive of any applicable VAT.


Children's Community Services for NHS Bedfordshire Clinical Commissioning Group, Central Bedfordshire Council and Bedford Borough Council.

Lot 1: — Children's Community Health Services — for Bedfordshire Clinical Commissioning Group, Bedford Borough Council, and Central Bedfordshire Council

The Children's services need to work closely with Education and Youth Offending Services.

The service delivery model is different from adults' services and the outcomes identified differ.

This lot covers the supply of the below services and has an initial funding envelope of 13 391 000 GBP per annum, inclusive of any applicable VAT, for the services to be provided. The Lot has the potential to grow by a further 6 270 000 GBP per annum inclusive of any applicable VAT in subsequent years as the future services listed are added.

Please note that whilst there is an intention to add the future services, the timing and extent (if at all), of addition of these future services will be at the sole discretion of the commissioning organisations and cannot be guaranteed.

Initial Chilidrens services will include:

Childrens Intermediate Care, Children LAC, Childrens OT, Nutrition and Dietetics Children (Includes Luton ), Childrens Community Nursing, Childrens Specialist Nursing, Community Paediatrics, Children's Eye service (includes Adults service), Children's continence service, Speech & Language — Children's, Children's Safeguarding, Health Visiting (0-5), Community Dental Services, School Nursing, Children and Young People's Drug and Alcohol Service, Wheelchair service (includes Adults Service)., Youth Offending Nurse.

Future potential Childrens services Include:

Integrated Discharge Planning/TB Service, Palliative Care Service,Special Needs Nursing, Audiology services, Childrens Community Nursing, Rapid Response service — early response to avoid A&E / hospital admissions, Assessment and coordination for children with neurodevelopmental presentations,

Long Term Conditions management support (to include respiraty, epilepsy, autism management, diabetes).

Adult's Community Services for NHS Bedfordshire Clinical Commissioning Group and Bedford Borough Council.

Lot 2 — Adults Community Health Services — for Bedfordshire Clinical Commissioning Group and Bedford Borough Council

The Adults' service provision has been divided into two lots to recognise the different demographic make-up, the geographic variation and the location of hospitals which may offer opportunities for different types of service model to deliver Community Health Services.

Lot 2: Adults Community Health Services — for Bedfordshire Clinical Commissioning Group and Bedford Borough Council (This Lot).

Lot 3: Adults Community Health Services — Bedfordshire Clinical Commissioning Group and Central Bedfordshire Council.

The supply of Adult Community Services for Lots 2 and 3 have a total initial funding envelope of £24,851,600 per annum, inclusive of any applicable VAT, for the services to be provided.

These Lots (2 & 3), have the potential to grow by a further £50,500,000 per annum inclusive of any applicable VAT in subsequent years as the future services listed are added.

It is expected that the split of the value of all services between lots 2 & 3 will be approximately as follows:

Lot 2 — Initially 9 600 400 GBP and then approximately 40 % of future growth values.

Lot 3 — Initially 15 251 200 GBP and then approximately 60 % of future growth values.

However the commissioners reserve the right, at their sole discretion, to move service lines, and the appropriate funds between lots 2 and 3.

This is likely be dependent on operational and efficiency reasons and can be raised at the ITPD /ITCD stage of the procurement.

Initial Adult services will include: Clinical Care at Home, Community Matrons, Complex Care Team, Discharge Planning, District Nursing, Geriatrician, Single Point of Access, Acquired Brain Injury, Chronic Fatigue service, Continence, Heart failure, Nutrition and Dietetics, Palliative care, Parkinsons Service, Podiatry, Podiatric Surgery, Foot health — podiatry, TB Nursing, Wound Care, Rapid Intervention, Rehabilitation and Therapy, Community Beds, Speech and language therapy. Rare Neurological Conditions service.

Future potential Adult services Include: Domiciliary Phlebotomy (not community caseload), Integrated COPD Rehab service, Integrated Community Diabetes Service, Continuing Healthcare — initiatives to support prevention of escalation, Complex Care Team, Care Homes, Domiciliary Care, Occupational Therapy service — (a wider range of services, including expanded OT assessments), Day Care, Falls Service, Telecare, Home care for sheltered housing, Reablement/Step Up/Step Down, Preventive services ( to help maintain independence and well-being), Centralised Community Phlebotomy, Imaging services (including: Ultrasound, x-ray, mobile MRI), Outpatients clinics (including: Community Geriatrics, Gynaecology, ENT, Opthalmology, some aspects of respiratory), Home Visiting service, Chronic Disease Management, Complex case management, Domiciliary physiotherapy services, GP APMS.
