Description:
Performing specialized laboratory tests along with the interpretation of the results

1. The subject of the contract is the provision of services in the field of specialized laboratory tests along with the interpretation of results under the project "Exosomes as a potential biomarker for monitoring and prognosis of transplanted kidney rejection".

2. A detailed description of the subject matter of the contract is provided in Annex 3 to the SWZ.

3. Detailed provisions regarding the performance of obligations relating to the implementation of the subject of the contract, payment terms are included in the model contract – Annex 4 to the SWZ.

1. The tender must be accompanied by:

(a) ESD statements

b) statement under Article 5k of EU Regulation 833/2014 and Article 7(1) of the Act on special arrangements for counteracting support for aggression against Ukraine and serving to protect national security – Annex 5 to the SWZ,

c) if applicable – a statement under Article 117(4) of the Public Procurement Law and an obligation of the entity providing resources

d) if applicable, a power of attorney to submit a bid, provided that the offer is submitted by a representative,

e) if applicable – a document confirming the appointment by the Contractors jointly applying for the contract, of a proxy to represent them in the procurement procedure or to represent them in the procedure and conclusion of the public procurement contract.

Contractors who are not subject to exclusion from the procedure pursuant to Article 108(1) and Article 109(1)(4) of the PPL may apply for the award of a contract.

To confirm that it is not subject to exclusion from the proceedings, the Contractor is obliged to submit together with the offer a current statement in the form of a Single Procurement Document – Annex 2 to the SWZ – in the form of

electronically bearing a qualified electronic signature and a copy or information from the National Court Register or from the Central Register and Information on Economic Activity, to the extent specified

in Article 109(1)(4) of the PPL, drawn up not earlier than 3 months before its submission.

2. Before selecting the most advantageous offer, the Contracting Authority shall call upon the Contractor whose bid has been rated the highest to submit within the prescribed deadline, not shorter than 10 days, valid as at the date of submission

1) entry in the register of diagnostic laboratories kept by the National Council of Laboratory Diagnosticians,

2) information from the National Criminal Register within the scope specified in Article 108(1)(1), (2) and (4) of the PPL, drawn up not earlier than 6 months before its submission,

3) a copy or information from the National Court Register or from the Central Register and Information on Economic Activity, within the scope specified in Article 109(1)(4) of the PPL, prepared not earlier than 3

months before its submission, if separate provisions require entry in the register or records,

4) the Contractor's statement on the lack of belonging to the same capital group (within the scope of Article 108 section 1 point 5 of the PPL), within the meaning of the Act of 16 February 2007 on competition and consumer protection (Journal of Laws).

together with documents or information proving that a tender, a partial tender or a request to participate has been prepared independently of another economic operator belonging to the same group

5) the Contractor's statement on the validity of the information contained in the statement referred to in Article 125(1) of the PPL, with regard to the grounds for exclusion from the proceedings indicated by the Contracting Authority, which

referred to in Chapter VII of the SWZ and in:

a) Article 108(1)(3) of the PPL,

b) Article 108(1)(4) of the PPL, concerning the prohibition of applying for a public contract

as a precautionary measure,

c) Article 108(1)(5) of the PPL, concerning the conclusion of an agreement with other contractors aimed at

the object of distortion of competition,

d) Article 108(1)(6) of the PPL
