1. The subject of the contract are the supply of oncological drugs, specified in parts for the Fryderyk Chopin Clinical Provincial Hospital No. 1 in Rzeszów.

2. Detailed description of the object of the contract, requirements of the Ordering Party, information on the acceptance of other quantities in the packaging, are specified in the Price and Tender Forms — Annex No 1 for individual parts, which are an integral part of the SWZ.

3.The Ordering Party requires offer items of the order that meet the requirements described in SWZ.

4. The scope of the contract includes the delivery of the items in the quantities specified in the Price and Offer Forms for the period of:

- Hello from No.1 to No. 4 — 12 months.

5.The Contracting Party shall not allow the submission of equivalent tenders.

Planned delivery of order items: for 12 months, counting from the date of conclusion of the contract.

The Contracting Authority shall allow the modification of the term of application of the contract concluded with the Contractor in the event that the whole subject of the contract is not fulfilled during its duration. In this case, the amendment of the contract requires the signing of an annex, provided that the Contracting Authority unilaterally notifies the Contractor of the proposed extension of the contract before the expiry of the time limit for which it was concluded.
