Services in the field of sanitary transport for the needs of the Multidisciplinary City Hospital of J. Ostrich in Poznan

1. The subject of the contract is the provision of sanitary transport services for the needs of the Multidisciplinary City Hospital named after J. Strusia in Poznań provided using specialized means of transport:

- medical ambulance (1 vehicle), properly equipped with certified (passported) medical equipment (i.e. at least: defibrillator, transport respirator, oxygen source, ECG device, cardiomonitor, pulse oximeter, infusion pump, BIO-BAG chamber for transporting infectious patients according to with the European standard), with the rescue team;

- medical ambulance (3 vehicles) with the requirements as above for requesting by phone every day of the week. An ambulance should be available for each telephone call by the customer, not later than 30 minutes from the call;

- an ambulance with a doctor on an emergency call from the Hospital Emergency Department at ul. Swiss 3, not later than 60 minutes from the request.

- an ambulance at the disposal of emergency care,

- medical ambulance (1 vehicle), properly equipped with certified (passported) medical equipment (i.e. at least: defibrillator, transport respirator, oxygen source, ECG device, cardiomonitor, pulse oximeter, infusion pump, BIO-BAG chamber for transporting infectious patients according to with the European standard), with the rescue team;

- medical ambulance (3 vehicles) with the requirements as above for requesting by phone every day of the week. An ambulance should be available for each telephone call by the customer, not later than 30 minutes from the call;

- an ambulance with a doctor on an emergency call from the Hospital Emergency Department at ul. Swiss 3, not later than 60 minutes from the request.

The contractor is obliged to take out civil liability insurance at his own expense and provide professional services. Appropriate marking of vehicles, having the right to use emergency vehicle signals. Vehicles should be technically sound, have required by law certificates of entry into service and performance of ordered transports. The services will be performed with ensuring the safety of persons carried by persons who are fully qualified to perform their activities.

- an ambulance at the disposal of Emergency Assistance, vehicles intended for the provision of services at the contracting authority must meet the PN-EN 1789 standards and the current requirements of the National Health Fund in accordance with the regulation of the President of the NFZ and have been reported and are under the control of the appropriate sanitary and epidemiological station.

The contractor is obliged to take out civil liability insurance at his own expense and perform professional services. Appropriate marking of vehicles, having the right to use emergency vehicle signals. Vehicles should be technically sound, have required by law certificates of entry into service and performance of ordered transports. The services will be performed with security for people being carried by people who are fully qualified to perform their activities.

2. List of transport needed and working time: Hospital, ul. Swiss 3 - SOR:

- an ambulance with a rescue team from Monday to Saturday from 9:00 to 17:00;

- an ambulance with a rescue team only for telephone demand every day of the week, not later than 30 minutes after the call;

- an ambulance with a doctor on an emergency call from the Hospital Emergency Department at ul. Swiss 3, not later than 60 minutes from the request.

Ambulances should be at the disposal of the customer in the hospital at ul. Swiss 3 if necessary on any other day of the week and at any hour after prior telephone notification of the demand in advance - also with a doctor. The ambulance standard should comply with European Union standards.

Emergency Aid of the Hospital at ul. Swiss 3 in Poznań:

- An ambulance with a driver available from Monday to Friday from 18:00 to 8:00; Saturdays, Sundays and holidays around the clock.
