Description:
Supply of disk arrays and expansion of the SAN HP SN8000B switch for the Center of Health Information Systems.

1. The subject of the order is the delivery of disk arrays and extension of the SAN HP SN8000B switch for the Center of Health Information Systems with headquarters in Warsaw at ul. Stanisława Dubois 5A.

2. A detailed description of the subject of the contract is attached as Annex 1 to the ToR - Description of the subject of the contract.

3. The Contractor is obliged to perform the contract on the terms and conditions described in the contract template constituting Annex 2 to the ToR.
