1. The subject of this order is the successive delivery of Tisagenlecleucel for the Oncology Center of the Institute. Maria Skłodowska-Curie Branch in Gliwice, hereinafter referred to as the contracting authority.

2. Description of the subject of the contract and the conditions of the contract are included in the forms:

a) Assortment and price specification - Annexes 2.1 to the ToR,

b) Contract template - Annex 4 to the ToR.

3. The Awarding Entity does not envisage concluding a framework agreement, establishing a dynamic purchasing system, electronic auction and supplementary orders.

2. Drugs that are the subject of the order must be approved for use in Poland in accordance with the provisions of the Pharmaceutical Law of September 6, 2001 (i.e. for marketing in the Republic of Poland for the offered medicine and present them at the request of the customer.
