1. The subject of the order is the expansion of the IBM Storwize V7000 disk array by 2 controllers, expansion enclosure and 130 TB of physical capacity (SSD NVMe / SSD / FCMs) for the Center of Health Information Systems based in Warsaw at ul. Stanisława Dubois 5, POLAND.

3. The Contractor is obliged to perform the contract on the terms and conditions described in the contract template constituting Annex 2 to the ToR.
