1. The subject of this order is the purchase of an HDR brachytherapy apparatus for the Oncology Center of the Institute Maria Skłodowska-Curie branch in Gliwice.
2. Description of the subject of the contract and the conditions of the contract are included in the forms:
a. description of the subject of the contract, required and assessed technical parameters of the subject of the contract, assortment and price specification - Annex 2 to the ToR;
b. model agreement - attachment 4 to the ToR.
3. The Awarding Entity does not envisage concluding a framework agreement, establishing a dynamic purchasing system, electronic auction or supplementary orders.

5. The purchase of the HDR brachytherapy apparatus is carried out as part of the National Program for Combating Cancer: 'Retrofitting radiotherapy facilities' in 2019 (replacement of HDR apparatus).

2. Description of the subject of the contract and the conditions of the contract are included in the forms:
a. description of the subject of the contract, required and assessed technical parameters of the subject of the contract, assortment and price specification - Annex 2 to the ToR;
b. model agreement - attachment 4 to the ToR.
3. Implementation of the order includes min. the following stages, which implementation and acceptance will take place at the latest on the following dates:
1) stage I - includes the delivery of equipment to the Brachytherapy Plant by 12/11/2019,
2) stage II - includes the installation and commissioning of equipment as well as basic and technical training of the Employer's personnel by 31.8.2020.
Section II.2.7 of this announcement gives the deadline for completing stage II as the deadline for completing the contract. The contractor is required to deliver the subject of the contract by 12/11/2019 in accordance with the stages indicated above and the provisions of the contract.
4. The Awarding Entity, prior to awarding the contract, to confirm that the offered subject of the contract meets the requirements specified by the Awarding Entity will call the Contractor whose bid has been best rated to be submitted within a designated period of not less than 10 days, valid on the date of submission of the declaration of conformity or CE certificate. for the camera offered.
5. The document referred to in item 4 should be submitted in the original in the form of an electronic document or in an electronic copy of the document certified as being true to the original. A document drawn up in a foreign language should be submitted together with a translation into Polish.
6. Employer, pursuant to art. 147 section 1 of the Public Procurement Law, requires the Contractor whose offer was considered the most advantageous to provide a performance bond in the amount of 5.00% of the gross price of the offer.
7. The purchase of the HDR brachytherapy device is carried out as part of the National Program for Combating Cancer: 'Retrofitting radiotherapy facilities' in 2019 (replacement of HDR devices).
