Description:
Delivery of workwear and individual protection products for Military Unit No. 6021.

The procedure is conducted in the form of an open tender in accordance with Article 132 of the PPL, awarded in two parts.

The subject of the contract is the supply of workwear and personal protective equipment divided into 3 (three) parts.

A detailed description of the subject of the contract is specified in: attachments no. 1, 1.1. - 1.3., 1.4. - 1.6. and No. 5 to the SWZ, i.e.: Offer form with the Assortment and Price Specification, Detailed description and requirements for marking and packaging the subject of the contract and Draft provisions of the contract.

If the evidence required in accordance with the Contract Notice is not submitted or is incomplete, the CUSTOMER shall call for their submission or supplementation.

Supply of workwear and personal protective equipment.

The description of the subject of the contract is specified in: attachments no.: 1, 1.1.-1.3., 1.4.-1.6. and No. 5 to the SWZ, i.e.: Offer form with the Assortment and Price Specification, Detailed description and requirements for marking and packaging the subject of the contract and Draft provisions of the contract.

The CONTRACTING AUTHORITY requires submission of the following means of proof together with the offer:

1) declarations of conformity confirming that the objects of the contract meet certain requirements, issued on the basis of Regulation (EU) 2016/425 of the European Parliament and of the Council of 9 March 2016. The declarations of conformity referred to above should be prepared in accordance with the Act of 13 April 2016 on conformity assessment systems and market surveillance (Journal of Laws of 2022, item 1854).

2) a detailed description of the offered subject of the contract, confirming the fulfillment of parameters (in particular: composition, type, weight of the fabric) and other requirements of the Ordering Party – applies to all offered items.

3) photos, brochures and/or catalogues with descriptions in Polish. The subject of the contract should be clearly presented in the prospectus and/or catalogue and/or in a photograph, in color, giving the opportunity to compare and check the description of the offered subject of the contract with attachments No. 1.4.–1.6. describing in detail the subject of the contract. At the same time, it should be marked (e.g. with a highlighter) in the prospectus and/or catalogue and/or in the photo, the proposed patterns of the subject of the contract – applies to all offered items.

4) samples with dimensions of min. 15x15 cm, confirming the quality of the basic materials from which the objects of the order will be made, confirming compliance with the parameters (in particular: colors, shade, raw material composition, in which the names of textile fibers and their percentage and material weight are given). Samples should be clearly described, giving the opportunity to compare and check the description of the offered subject of the contract with Annex 1.5. describing in detail the subjects of the contract – applies to the offered items from part II items 1-4, 6-7, 10-13 and 16 - Annex 1.2.

5) patterns of offered items, confirming the fulfillment of parameters (in particular: colors, composition, meeting standards). Patterns in the form of individual items (one glove from a pair) should be clearly described, giving the opportunity to compare and check the description of the offered subject of the contract with attachments No. 1.4. – 1.6. describing in detail the subjects of the contract – applies to the offered items: Part I items 6–19 – Annex No. 1.1., Part II item 8 – Annex No. 1.2., Part III items 3–4 – Annex No. 1.3.

6) patterns of offered items, confirming the fulfillment of parameters (in particular: composition, meeting standards, functionality). Patterns in the form of individual items in any color should be clearly described, giving the opportunity to compare and check the description of the offered subject of the contract with Annex No. 1.6. describing in detail the subjects of the contract - applies to the offered items: Part III, items 11-16 - Annex 1.3.

The descriptions of the samples and models referred to above should contain the following information: the name, address and brand name of the manufacturer; country of manufacture; the raw material composition, in which the names of textile fibres, their percentage and the weight of the material are given – if required; CE mark – if required; the number of the relevant EN standard – if required; marking with the symbol of the appropriate protection category – if required.

Submitted descriptions, samples and formulas will be evaluated in the category: "meets-does not meet" the requirements of the Ordering Party. The Contracting Authority reserves the right to request the Contractor to present the submitted samples and samples of the subject of the contract. The presentation will take the form of a visual inspection to confirm the fulfillment of the parameters. The time for presenting samples and formulas is set by the CUSTOMER at 25 minutes.
