1. The subject of the contract is: purchase and supply of reagents and lease of analyzers to perform tests in the field of general analytics for medical entities of the Podkarpackie Voivodeship listed in Chapter I SWZ.

2. The description of the subject of the contract is contained in Appendix No. 4 to the SWZ Detailed description of the subject of the contract (SOPZ) and Appendix No. 3 to the SWZ. Draft provisions of the contract.

The implementation of the subject of the contract will take place in quantities, assortment and to the Ordering Parties indicated in Annex 4.SOPZ to the SWZ.

To confirm the compliance of the offered deliveries with the requirements of the Ordering Party, the Ordering Party shall request the following means of proof:

Catalogues, instructions, brochures (in Polish) of the offered subject of the contract confirming compliance with the technical and evaluated requirements specified by the Ordering Party in Appendix 4.3. The form of technical parameters required, and in Appendix No. 4.4. Form of evaluated parameters. The materials should indicate which item they concern.

In the event that a given parameter described by the Contracting entity in the above-mentioned Annexes is not confirmed in the materials referred to in the previous sentence, the Contracting Authority allows confirmation of a given parameter / requirement by submitting a statement to the Contractor.

REMARK! Completing the technical table is not tantamount to submitting this statement, it must be submitted separately.
