1. The subject of the contract is a joint order for the supply of personal protective equipment for units of the Lower Silesian Voivodeship - 10 lots.

2. A detailed description of the subject of the contract are price attachments for each part - Annexes No. 1.1 to 1.10 to the SWZ. The conditions for its implementation have been specified by the Contracting Authority

in the essential provisions of the agreement, constituting Annexes 7 to the SWZ.

3. The Ordering Party requires submission of the said means of proof (TOGETHER WITH THE OFFER), i.e. certificates and/or declaration of conformity and/or a photo of the product in relation to the assortments indicated in the price forms appropriate to the given task. The contracting authority does not provide for supplementing the evidence in question.
