1. The subject of the contract is the provision of sanitary transport services for the Provincial Specialist Hospital No. 4 in Bytom divided into two packages (two parts):
- Package 1 — sanitary transport of patients with dysfunction of the motor organ;
- Package 2 — sanitary transport of dialysis patients.
2. The subject of the order is described in detail in the Subject Descriptions of the Order, which are Annexes No. 2A and 2B to SWZ respectively.
