The subject of this contract is the provision of maintenance services for the TOMO HD Accuray tomotherapy apparatus s / n 0210376 for Zakład Radioterapia Centrum Onkologii - Instytut im. Maria Skłodowska-Curie - the branch in Gliwice.

1. The subject of the contract includes:

1) periodic technical inspections and equipment maintenance,

2) maintenance services to remove critical failures,

3) other maintenance services,

4) one-time device repair consisting of replacing the magnetron and water pump.

2. The contractor will be obliged to:

1) performing a one-off device repair consisting in replacing the magnetron and water pump, within 30 days from the date of the contract,

2) rendering other services that are the subject of the contract within 24 months, counting from the date of confirmation of the one-off repair performance as above.

3. A detailed description of the subject of the contract is attached as Annex 1 to the Contract Template (ie Annex 5 to the ToR).
