1. The object of the order is the purchase and delivery of a medical accelerator carrying out a helic technique with integrated imaging of a fan beam with equipment, as described in the technical requirements of the accelerator in Annex 2 to SWZ.

The implementation of the construction and technological project and the execution of construction works related to the installation of the accelerator — in accordance with the functional and utility program of Annex 3 to the SWZ.

The subject of the contract includes:

1.1. disassembly of the accelerator possessed by the contracting operator with its disposal;

1.2. implementation of the construction and technological project of adaptation and repair of the bunker and control room V5 of the Radiotherapy Department I in NIO-PIB;

1.3 the execution of construction works;

1.4 delivery, assembly, installation, commissioning and commissioning of the accelerator clinical operation;

1.5 training of the technical and medical personnel of the contracting officer.

Measures of evidence in question — according to Chapter. XV SWZ.

Where the contractor has not submitted the measures of evidence in question or the measures of evidence submitted are incomplete, the contracting officer shall summon the contractor to submit them or supplement them within the time limit set, except where the measure of evidence in question serves to confirm compliance with the characteristics or criteria set out in the description of the criteria for evaluation of tenders or, despite the submission of the evidence in question, the offer is either subject to rejection or there are indications of annulment of the proceedings.
