The subject of the contract is the supply of tests for testing blood-borne virological markers and syphilis markers: HBs antigen, anti-HCV, anti-HIV ½ and anti-Treponema pallidum, together with consumables, consumables, calibrations and controls as well as the lease of complete equipment necessary to perform the above tests in accordance with the assortment and price form - Annex 2 to IdW. The order should be carried out for a period of 36 months from the date of signing the contract.

2. The offered object of the contract must meet the requirements of the Employer and be ready to use as intended.
The offered object of the contract must be authorized for trading and use as a medical device (only for medical devices).
3. The Awarding Entity does not provide for the requirements referred to in art. 29 paragraph 3a (employment under an employment contract).
4. The Purchaser does not allow the submission of partial offers.
5. The Employer does not reserve the obligation for the Contractor to perform key parts of the contract personally.
6. The Awarding Entity does not provide for the requirements referred to in art. 29 paragraph 3 of the Act (employment under an employment contract)
7. The Purchaser does not allow the submission of variants.
8. The Awarding Entity does not provide for the possibility of awarding supplementary contracts referred to in art. 67 section 1 point 7 of the PPL Act.
9. The Awarding Entity does not envisage concluding a framework agreement.
10. The Awarding Entity did not conduct technical dialogue.
11. The Awarding Entity does not provide for reimbursement of the costs of participation in these proceedings.
12. The proceedings are conducted in Polish.
13. The Purchaser does not provide for settlement of the order in foreign currencies.
