The subject of the contract is: Supply of server software and mobile application along with training from the IT system and delivery of equipment for home rehabilitation monitoring – project titled 'Assessment of the impact of telerehabilitation for cardiovascular and respiratory conditions on overall fitness and quality of life of patients with pulmonary arterial hypertension' (contract no. 2024/ABM/01/00069).

1) Stage I - Delivery and implementation of a system for recording patients with the capability of remotely assigning rehabilitation tasks, remotely supervising patient activity, and aggregating data from medical devices, consisting of server software and mobile applications for iOS and Android.
2) Stage II – Delivery of medical equipment, including research infrastructure – gradual delivery.
3) Stage III – Training at the Ordering Party's site in the city of Wrocław and at a maximum of 7 other centers in Poland at a time and place designated by them.
4) Stage IV - Maintenance of the patient recording system with the capability of remotely assigning rehabilitation tasks, remotely supervising patient activity, and aggregating data from medical devices, consisting of server software and mobile applications for iOS and Android. The subject of the contract has been described in detail in the Description of the subject of the contract (attachment no. 1 to the SWZ), Bid Form (attachment no. 2 to the SWZ), Price and assortment form (attachment no. 3 to the SWZ). The Ordering Party requires that the offered equipment (provided under Stage II of the task execution) meets the requirements specified in the Price and assortment form. Detailed terms and conditions of contract execution are defined in the Contract template (attachment no. 5 to the SWZ).
4. The Ordering Party states the amount intended for financing the order, which is gross: 1,507,000.00 PLN.
