The subject of the contract is the purchase and delivery:
medical systems divided into:
Package No. 1 – intraoperative parathyroid identification system,
Package No. 2 – endoscopic capsule system for the small intestine,
Package No. 3 – system for assessing screening mammography AI program,
capsules for the small intestine for the endoscopic capsule system of the small intestine from Package No. 2 – Package No. 2A
in accordance with Appendix No. 2 to the SWZ – Price Offer - Detailed description of the subject of the order.
Concerns Packages No. 1, 2, 3
The order also includes:
1. installation and commissioning,
2. delivery of software with indefinite licenses,
3. training of personnel in terms of operation and practical use of the system, after prior written agreement of the date with the Ordering Party, completed by signing the “Training Protocol”;
4. handover for use along with all required approvals, certificates,
5. 24-month warranty counted from the date of signing by authorized representatives of the Ordering Party and the Contractor the “Final Acceptance Protocol” regarding the subject of the order,
6. a guarantee of 36 months of post-warranty service within which the Contractor provides in the offer price the system service, including:
a) inspections, if required by the manufacturer,
b) repair and replacement of parts with factory new ones,
c) all repairs, inspections, maintenance along with spare parts and materials required for their execution,
d) extension of warranty for the time of system failure, in the case of warranty repairs,
e) total system failure (in case of breakdown/failure) will not exceed 20 working days over periods of 12 months, failure means lack of system functionality for more than 8 hours a day,
f) response time of service to the report with the undertaking of repair (not longer than 48h on working days i.e. from Monday to Friday, excluding public holidays) counted from the date of the failure report, including also the fault referred to in Appendix No. 2 to the SWZ in Package No. 3.
Concerns Package No. 2 A – detailed conditions for implementation are specified in Appendix No. 6C to the SWZ – Draft provisions of the contract.
The Ordering Party reserves the right to cancel the proceedings based on Article 257 of the Pzp.
The Ordering Party allows for the submission of partial offers for complete Packages No. 1 and No. 3.
The Ordering Party does not allow for the submission of partial offers concerning Package No. 2 and Package No. 2 A.
The division of the order in this case is not possible due to the compatibility of the system with the capsule.
The Ordering Party does not allow for the submission of variant offers.
