3.1. The subject of the order is the delivery of medical equipment along with IT infrastructure: 3.1.1. a computer tomograph along with installation, configuration, commissioning, and project for fixed shielding; 3.1.2. IT infrastructure: Server I - 1 piece, Server II - 1 piece, backup software - 1 piece, matrix - 1 piece, NAS server - 1 piece, network switch - 2 pieces, uninterruptible power supply - 2 pieces, automatic CD duplicator (including control computer) - 1 set. 3.2. The subject of the order also includes connecting the offered device to the existing PACS/RIS system of the Alteris company in the hospital in a way that ensures full integration, including the transfer of diagnostic images to and from the digital PACS archive and the implementation of the DICOM WORKLIST function. Should the connection to the current system require it, the Contractor agrees to provide the necessary licenses, such as additional licenses for the laboratories in the Alteris system, as well as integration with the Client's HIS system, AMMS Asseco. In addition, the contractor is obliged to perform communication tests to ensure the correct and seamless operation of all integrated components. 3.3. If necessary, the Contractor is responsible for connecting the computer tomograph and server equipment and other delivered devices to the Client's telecommunication network in accordance with the guidelines and requirements specified by the Client's IT Section. The connection of the device to the hospital network must comply with the applicable standards and network protocols and ensure an appropriate level of security. If needed, the Contractor is obliged to provide a secure VPN (Virtual Private Network) connection for remote access to the devices and systems related to the delivered computer tomograph and server equipment. The Contractor must adhere to security standards including VPN protocol standards, data encryption standards, and device certification standards. The VPN connection must be managed by designated employees of the hospital's IT section. Each VPN connection must be reported by the Contractor. The Contractor commits to close cooperation with the hospital's IT section to configure, test, and maintain the VPN connection in accordance with the hospital's requirements. The Contractor will provide all necessary technical information and documentation regarding VPN connections. The Contractor commits to regularly update the software and firmware of the devices, including the computer tomograph and server equipment, according to the manufacturer's guidelines and those of the hospital's IT section. The Contractor will regularly inform the hospital's IT section about planned updates related to devices connected to the network to ensure the highest level of IT security. In the event of detecting any IT security threats, the Contractor will immediately notify the hospital's IT section. The Contractor shall comply with all IT security requirements specified by the hospital's IT section, including requirements regarding passwords, encryption, and antivirus protections. The Contractor shall ensure the protection of privacy and confidentiality of personal data of individuals using the connection in accordance with applicable legal regulations. The Contractor will only transfer personal data of individuals using the connection in cases provided for by applicable legal regulations or with the explicit consent of the data subject. The Contractor will adhere to the hospital's privacy and data protection policy and the applicable regulations concerning personal data. 3.4. A detailed description of the requirements set by the Client that the offered equipment must meet regarding parameters, configuration, and equipment is specified in Appendix No. 1 to the SWZ - Description of the subject of the order/Subject Specification TK/Description of minimum parameters (hereinafter referred to as “OPZ”), as well as Appendix No. 4 to the SWZ - Proposed contractual provisions. 3.7. The parameters of the subject of the order presented in Appendix No. 1 to the SWZ constitute the minimum technical and quality requirements set by the Client. In the event of offering equipment, apparatus or assortment that does not meet the minimum technical, operational, usable, and quality parameters specified in Appendix 1 to the SWZ, the Contractor's offer will be rejected. 3.8. The delivered equipment and devices must: • be factory new, unused, not pre-packaged, not previously exhibited, free from physical and legal defects, fully operational and ready for use; • have the appropriate certifications, certificates, and other documents prescribed by law and meet all requirements of applicable standards set by the law; • be authorized for trade in accordance with general product safety regulations. 3.9. The offer must be clear and comprehensive, i.e. it must cover the entire range of the object of the order. 3.10. Place of execution: Medical Care Center in Jarosław, ul. 3 Maja 70, 37-500 Jarosław. 3.11. Required warranty period: in accordance with Appendix No. 1 to the SWZ, counted from the day the final acceptance protocol is signed by both parties.
