Supply of endoscopic equipment and accessories as part of the investment "Construction of a hospital building for the central operating block and surgical departments with the reconstruction of existing buildings."
1. The subject of the contract is the supply of endoscopic equipment and accessories as part of the investment "Construction of a hospital building for the central operating block and surgical departments with the reconstruction of existing buildings."
2. The subject of the contract includes:
1) Bronchofiberscope – 1 set,
2) Videoduodenoscope – 1 set,
3) Bronchofiberscope with washer – 1 set,
4) Endoscopic medical cart – 2 sets,
5) Monitor – 2 sets,
6) Endoscopic pump – 2 sets,
7) Videogastroscope – 3 sets,
8) Image processor – 2 sets,
9) Videocolonoscope – 3 sets,
10) Automatic endoscopic washer – 1 set,
11) Penendoscope – 1 set,
12) Endoscopic vision system - 1 set,
13) Artificial intelligence system - 1 set,
14) Data archiving program - 1 set,
15) Ultrasonic cleaner - 1 set,
16) Two-door endoscopic sterilization cabinet - 2 sets,
17) Electrocoagulation device - 2 sets.
3. The scope of the subject of the contract includes:
1) delivery of brand new (production year: 2024/2025), coming from current production and unused prior to the day of delivery, with the exclusion of use necessary for conducting tests of correct operation, coming from the official distribution channels of the manufacturer, ensuring in particular the implementation of warranty rights of the equipment as specified in Annex No. 1 to the SWZ along with its transport, carrying, and installation (if applicable) and setting in the location indicated by the Ordering Party,
2) delivery, along with the order, of the required operation manual in Polish (if applicable),
3) delivery of technical and user documentation related to the subject of the contract, including:
a) user manual in Polish - 1 copy in paper version and/or 1 copy in electronic version,
b) shortened version of the user manual and safe use instructions, in the form of a laminated document (if applicable),
c) device passport,
d) communication instruction (to be hung) in Polish (if applicable),
e) warranty card with a list of authorized service points in Poland,
f) list of parts and consumables necessary for the current operation of the subject of the contract,
g) maintenance, cleaning, disinfection, and sterilization instructions (if applicable),
h) CE Certificate (if applicable) and Declaration of Conformity issued by the manufacturer,
i) list of service activities that can be performed by the user independently without loss of warranty,
j) test, examination, and measurement protocols as defined by regulations (if applicable),
k) access codes (approved by the manufacturer) necessary for the operation of the device and its service (if applicable)
l) statement from the manufacturer or authorized representative guaranteeing the availability of spare parts and consumables for a minimum of 5 years from the date of delivery of the device.
4) delivery, along with the order, of warranty documents issued by the manufacturer, as well as all necessary cables, wires, connectors, etc. (if required for proper functioning of the product),
5) cleaning of premises after installation and arrangement of the equipment,
6) conducting training on the operation of the equipment for the specified employees of the Ordering Party (regarding elements delivered for which such necessity is provided in the detailed description of the subject of the contract),
7) transport and insurance costs of the delivered equipment are borne by the Contractor,
8) all electrical elements of the offered equipment must meet the Polish power standard,
9) The Contractor will be responsible to the Ordering Party for being authorized to market the software installed in the offered equipment and for the fact that the Ordering Party will be entitled to use all software supplied along with the offered equipment as part of normal use upon entering into the agreement,
10) The Contractor is responsible for the entire order, including its course and timely execution, quality, compliance with technical, qualitative conditions and applicable legal regulations.
4. The detailed description of the subject of the contract, the size, scope, type and amount of deliveries, warranty conditions, service, and technical parameters are specified in Annex No. 1 to the SWZ and the draft agreement Annex No. 2 to the SWZ.
