Only a few processes have already been digitized and coordinated with each other via specific applications. Communication with upstream and downstream service providers (referring physicians/referring physicians, follow-up caregivers, rehabilitation and care facilities) usually takes place in an analogue way.
Within the framework of the present award procedure, the AG intends to implement a patient portal with an initial fixed term of three years, whereby it should be noted that the measure will be financed by funds from the Hospital Future Fund (funding element 2). In this respect, it must be stated that in order to achieve eligibility for funding, all mandatory requirements for the relevant funding event 2 must be met.
In addition to the initial fixed term, the AG's clinics are entitled to a two-time extension option. It can extend the term of the contract by one year at a time, i.e. up to a maximum contract term of 5 years, by unilateral written declaration.
