1. The subject of the contract is: purchase of medical equipment and apparatus for the needs of the Endoscopy Laboratory of the Urology and Urological Oncology Department of SP ZOZ Provincial Specialist Hospital No. 3 in Rybnik divided into 8 parts (hereinafter: part or package)
2. Detailed description of the subject of the contract is contained in the attachments:
1) Price form (separately for each package) – attachment No. 1.1 to the SWZ,
2) Technical specifications (separately for each package) – attachment No. 1.2.a-1.2.h to the SWZ,
3) Quality assurance and service conditions (separately for each of the packages) – attachment No. 1.3. to the SWZ,
4) Draft provisions of the contract – attachment No. 2 to the SWZ.
3. The subject of the contract is divided into 8 separate packages (parts), including:
1. Endoscopic tower with video tracking and diathermy - 1 set
2. Thulium laser with optical sets for laser enucleation and bipolar with morcellator - 1 set
3. Irrigation fluid tower - 1 set
4. Anesthesia machine - 1 set
5. Operating table - 1 set
6. Patient transport trolley in a lying position - 1 set
7. Cardiac monitor - 2 sets
8. Tooling equipment for the endoscopic tower - 1 set
4. The ordering party allows the possibility of submitting offers for selected parts, but does not allow the submission of offers for selected items in a given package.
5. Offers not containing the full scope of the subject of the contract in a given package will be rejected.
NOTE:
The ordering party requires pricing of the unit of measure indicated in the price form. Changes to the unit of measure are not allowed. The ordering party points out that introducing additional columns by the contractor in the price form may lead to ambiguity in the offer or the calculation of the offer price in a manner different from the description provided by the ordering party, and consequently - to the rejection of the offer.
6. The subject of the contract must be approved for circulation and use – in accordance with applicable law, and also have declarations of conformity (applies to all classes of medical devices) and certificates of compliance for the entire offered assortment issued by a notified body (applies to all classes of medical devices except class I).
The task has been financed from the budget of the Silesian Voivodeship.
The ordering party reserves the right, based on art. 257 of the Public Procurement Law, to cancel the procurement procedure if the public funds intended for financing all or part of the contract are not granted to them.
The ordering party informs that, in accordance with art. 139 of the Public Procurement Law, it will first conduct an examination and evaluation of the offers, and then carry out the qualification of the Contractor whose offer has been rated the highest, in terms of the absence of grounds for exclusion and fulfillment of the conditions for participation in the proceedings.
In fulfilling the obligation arising from Article 24(6) of the Act of June 14, 2024, on the protection of whistleblowers (Journal of Laws of 2024, item 928), the ordering party informs that an internal procedure for reporting breaches of law and taking follow-up actions has been adopted at the Independent Public Health Care Facility Provincial Specialist Hospital No. 3 in Rybnik.
