Description:
The tele-consultation platform aims to provide Sana Clinics with a nationwide digital support service for the consulting care of geriatric patients, facilitating virtual communication, documentation, and coordination of telemedicine services while ensuring interoperability with existing hospital information systems.

The tele-consultation platform is intended to provide Sana Clinics with a Germany-wide digital accompaniment and support service for the consulting care of patients with a focus on geriatric patients. This platform must enable virtual network structures and the associated patient-centered documentation, communication and coordination of telemedicine services. For the efficient use of the platform, standard interfaces must be established for the transmission of existing information to the patient. For example, it must be possible to open and fill a case file on the platform from the hospital information system. This procedure ensures that selected information is already available to the supply partners when the case file is opened. In addition, risk screening results generated on the platform, geriatric assessments of the patient and medical documents such as consultation reports can be automatically transferred to the primary system for service documentation and archiving. The framework agreement is a contract concluded for the term specified in the contract notice, which obliges the contractor(s) to perform the services called up with individual contracts under the conditions laid down in the framework agreement and the respective individual contract. These individual contracts shall be awarded exclusively by the contracting authorities listed in Annex 2 "Directory of contracting authorities" to the undertaking(s) which is or are contracting parties to the framework agreement at that time. The contracting authorities entitled to call are recipients of funding from the Hospital Future Act and are obliged to comply with public procurement law by the funding notice. As public contracting authorities within the meaning of the GWB, some contracting authorities are bound by (antitrust) procurement law.

In addition to the strategic topics of securing skilled workers and outpatient care, digitization offers immense opportunities to make care processes more efficient. In the future, we will have to focus on the early assessment of symptoms and the efficient distribution of resources. These measures make the supply more predictable and at the same time minimise the misallocation of resources. In addition, the early identification of complex care needs through screenings and risk assessments plays an important role, enabling targeted treatment when it is most urgent. In a world where medical expertise is constantly evolving, it will be crucial to have the right experts at your disposal. However, this is not always easy, especially in rural areas or for special issues. By networking subject matter experts, patients can benefit from a more comprehensive pool of knowledge. Collaboration between physicians, clinics, and other care partners, as well as the integration of patients and their families, is essential and leads to more effective coordination of patient care. The provision of specialist knowledge in the context of virtual case discussions, rounds and consultations enables the exchange between experts across regional borders, which improves medical care and makes more effective use of existing resources. Electronic Patient Records/Case Files (EPA/EFA) The platform solution can enable the centralized collection and management of a patient's health data, including medical findings, treatment history, medication plan, and allergies. Telemedicine and remote monitoring The platform solution can enable virtual consultations, consultations, and remote monitoring of patients. This is especially useful for patients in remote areas or with limited mobility. Remote monitoring devices can record and transmit health data such as blood pressure, blood sugar, and heart rate. c. Health Management and Prevention The Platform Solution can provide services to promote a healthy lifestyle, including nutritional counseling, fitness programs, and smoking cessation. You can also set personalized health goals and track progress to increase patient motivation and prevent disease. d. Interoperability and data exchange The platform solution can support the networking of different information systems and enable the seamless exchange of data between health and social service providers, hospitals, laboratories and pharmacies. This improves cooperation and communication in the healthcare system. e. Data analytics and artificial intelligence The platform solution can use advanced analytics techniques and artificial intelligence to analyze health data and gain insights. This can help detect diseases early, identify treatment patterns, personalize therapies, and optimize treatment plans. f. Medication Management The platform solution can support medication management, including electronic prescriptions, medication planning, and medication reminders. This helps reduce medication errors and improve adherence to therapy. The contractor must ensure the successful provision, introduction and support of the telekonsil platform in the participating hospitals of Sana Kliniken AG within the specified schedule and the binding offer price and will take over the planning, management and coordination of the project within its area of responsibility. The Contractor shall draw the Client's attention to all activities required by the Client. The contractor has overall responsibility and system responsibility. The project phase planned for the introduction of the tele-consultation platform within the framework of the KHZG envisages an introduction from the beginning of Q4/2024 at the latest and completion by Q2/2025 at the latest. This includes: - Provision of the required environment - Installation and testing - Training of users and site IT staff - Acceptance of the productive system and commissioning - Interface implementation and integration with the primary clinical workstation system on site
