Rheinland Klinikum Neuss GmbH has set itself the goal of adding a future-oriented portal solution for discharge and transition management to its existing IT systems in 2024. The portal solution, together with existing IT systems, must meet the relevant MUST criteria of KHZG funding element 2 (§ 19 para. 1 sentence 1 no. 2 KHSFV), as well as the associated CAN criteria if possible.

Procurement Discharge and Transition Management.
The subject of the contract to be awarded is the delivery and ready-to-use installation/provision (bringing about operational readiness) of a software-supported discharge and transition management as a cloud solution (SaaS), system service including hotline support over a period of at least 5 years.
