Procurement of an aftercare platform for digital discharge and transition management.
Within the framework of the present tender, the operational construction of an aftercare platform is to be offered to support seamless digital discharge and transition management in accordance with KHZG specifications. The aftercare platform must meet the following essential criteria:
▪ Fulfilment of the MUST criteria of the KHZG funding element 2 (discharge and transition management) in interaction with already existing systems (KIS Orbis)
▪ Fulfilment of all other KHZG framework conditions, in particular the requirements of § 19 (2) and (3) KHSFV
