1. The subject of the order is the supply of medical devices for the equipment of hospital departments and the operating room.
2. The subject of the order is detailed in Annex No. 1 to the SWZ - Offer Form, in Annexes No. 2.1 to 2.9 to the SWZ - Overview of required quality and technical parameters, and in Annex No. 5 to the SWZ - Draft Contract Provisions, which specify the quantity of the subject of the order and the description of the subject of the order.
3. The order is financed:
a) Package number 1-4 under the contract DZ-IV-A/12/2025 along with annex No. 1 concluded between the Provincial Combined Hospital in Konin and the Greater Poland Voivodeship for granting a subsidy for the equipment of the ENT Department along with the operating room;
b) Package number 5 under contract No. DZ-IV-A/68/2025 concluded between the Provincial Combined Hospital in Konin and the Greater Poland Voivodeship for granting a subsidy for the purchase of hemodialysis machines;
c) Package number 6 under contract No. DZ-IV-A/69/2025 concluded between the Provincial Combined Hospital in Konin and the Greater Poland Voivodeship for granting a subsidy for the purchase of a holmium laser for performing urological procedures;
d) Packages 7-8 under contract No. DZ-IV-A/70/2025 concluded between the Provincial Combined Hospital in Konin and the Greater Poland Voivodeship for granting a subsidy for the purchase of equipment for performing orthopedic surgeries on the knee and hip joints;
e) Package 9 from the hospital’s own resources.
4. The contracting authority allows the submission of partial offers. The subject of the order has been divided into parts (packages). Offers can be submitted for one, several, or all parts of the order.
