Description:
Delivery of medical equipment for anesthesiology and intensive care departments, including various monitoring and treatment devices, with the option for partial offers.

The subject of the order is: Delivery of medical equipment: Project co-financed under the "National Transplant Program. Purchase of Equipment and Apparatus" - through the purchase of equipment and apparatus for the departments of anesthesiology and intensive care with the highest donation activity in 2024.

Task 1 – Intracranial pressure ICP monitoring set for the Department of Anesthesiology and Intensive Care
Task 2 – BIS Monitor for the Department of Anesthesiology and Intensive Care
Task 3 – Bed with an anti-decubitus mattress for the Department of Anesthesiology and Intensive Care
Task 4 – Docking stations with a set of infusion pumps for the Department of Anesthesiology and Intensive Care
Task 5 – Hemodynamic platform for the Department of Anesthesiology and Intensive Care
Task 6 – Ultrasound device for the Department of Anesthesiology and Intensive Care

The contracting authority allows partial offers. The parts of the order are the individual tasks defined above, i.e., tasks from 1 to 6. The contracting authority informs that the contractor may submit an offer for a maximum of 6 tasks (without limiting the number of tasks for one contractor).

A detailed description of the subject of the order is included in Annex 2b and 5 to the Terms of Reference.

The contracting authority requires a realization of the subject of the order on the principles set out in the contract. The planned provisions of the contract regarding public procurement to be included in the contract constitute annexes to the Terms of Reference.

INFORMATION ABOUT ANTICIPATED ORDERS REFERRED TO IN ART. 214 SEC. 1 POINTS 7 AND 8, IF THE CONTRACTING AUTHORITY FORESEES THE AWARDING OF SUCH ORDERS The contracting authority does not anticipate awarding orders referred to in art. 214 sec. 1 point 7 of the PPL Act. The contracting authority does not anticipate awarding orders referred to in art. 214 sec. 1 point 8 of the PPL Act.

DESCRIPTION OF THE CRITERIA THAT THE CONTRACTING AUTHORITY WILL FOLLOW WHEN SELECTING THE OFFER, ALONG WITH THE WEIGHTING OF THESE CRITERIA AND THE METHOD OF EVALUATING OFFERS 13.1. The evaluation of offers will be carried out based on the criteria specified in the Terms of Reference and their weights. Offers will be rated numerically. During the evaluation process, offers under review will be awarded points for the following criteria based on the following principles: With regard to tasks no: 1 - 5: CRITERION PRICE weight: 60% CRITERION TIME OF CONTRACT EXECUTION weight: 40%

3.3. Requirements of the Ordering Party regarding the subject of the contract:
1. The offered medical product must have certificates, certifications, and be registered in accordance with the legal regulations requiring certifications, certificates, and registration,
2. The offered medical product must be introduced into circulation and use in accordance with the requirements of the Act of April 7, 2022, on medical products (Journal of Laws 2024, item 1620, as amended),
3. The offered medical product must meet the standards or technical specifications applicable to this product;
In the event that the offered medical product does not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor is obliged to carry out at their own cost and on their own behalf all actions necessary to fulfill all legal requirements related to the introduction of the product into circulation and use on the territory of the Republic of Poland. The Contractor is obliged to perform such actions within the deadlines resulting from applicable regulations and to provide the Ordering Party with relevant documents confirming the completion of such actions within these deadlines.
3.4. In cases where trademarks, patents, or origin, source, or specific process characterizing certain products or services delivered by a specific contractor have been indicated in the description of the subject of the contract, it means that the Ordering Party cannot describe the subject of the contract in a sufficiently precise and understandable way; however, in each such situation, the Ordering Party allows for equivalent solutions to those described in the above manner. In such situations, any references to trademarks, patents, origin, source, or specific process characterizing certain products or services delivered by a specific contractor should be read together with the words 'or equivalent'.
When describing the subject of the contract by reference to standards, technical evaluations, technical specifications, and systems of technical references, referred to in Article 101(1)(2) and (3), the Ordering Party allows for equivalent solutions to those described. In such situations, any references should be read together with the words 'or equivalent'.
3.5. The Ordering Party requires the implementation of the subject of the contract on the terms specified in the agreement. The draft provisions of the contract regarding the public procurement, which will be introduced into the contract, are annexes to the SWZ.
INFORMATION ON THE EXPECTED CONTRACTS, AS REFERRED TO IN ARTICLE 214(1)(7) AND (8), IF THE ORDERING PARTY EXPECTS TO AWARD SUCH CONTRACTS, the Ordering Party does not foresee awarding contracts referred to in Article 214(1)(7) of the Public Procurement Law. The Ordering Party does not foresee awarding contracts referred to in Article 214(1)(8) of the Public Procurement Law.
