The subject of the contract is the delivery of medical devices and medical furniture, which are medical products within the meaning of the Act of April 7, 2022, on medical products (Journal of Laws 2024, item 1620, as amended), hereinafter referred to as "devices" to the Health Care Institutions Team in Cieszyn. The order consists of 15 parts: Part No. 1 - Delivery of an anesthesia machine Part No. 2 - Delivery of endoscopic and laparoscopic systems - complete endoscopic tower with a set of tools - urology Part No. 3 - Delivery of endoscopic and laparoscopic systems - Operating rectoscope Part No. 4 - Delivery of a device for safe and contactless dispensing of formaldehyde with a starter kit Part No. 5 - Delivery of an anesthesiologist trolley Part No. 6 - Delivery of a set of cystoscopes with a visual track Part No. 7 - Delivery of an optical urethrotome Part No. 8 - Delivery of surgical diathermy with equipment Part No. 9 - Delivery of urodynamics Part No. 10 - Delivery of a laryngological unit Part No. 11 - Delivery of a nasofiberscope with a light source Part No. 12 - Delivery of a table for medical equipment Part No. 13 - Delivery of a toilet chair Part No. 14 - Delivery of equipment for CT machine Part No. 15 - Delivery of equipment for X-ray machine The assortment divided into parts along with the anticipated demand is included in Attachment No. 2 to the SWZ - Price Form. The Ordering Party allows partial offers to be submitted. Offers may be submitted for one or more parts. The Ordering Party will choose the Contractor for each part separately.
