The subject of the contract is the provision of patient nutrition services both on weekdays and holidays, for a period of 36 months from 1 September 2022. The term "patient nutrition" should be understood as the contractor's own preparation (cooking) and delivery of ready meals to individual buildings of the Ordering Party, in accordance with the orders placed by the Ordering Party and at the times specified by the Ordering Party. Preparation of meals will take place at the catering point organized in the "Zimowit" building. Serving meals to patients undergoing treatment at the Ordering Party will take place in the dining room, organized in the buildings "Zimowit", "Eskulap", "Maria", "Teresa", "Polonia", "Świtezianka", "Gołąbek" and "Anna". The ordering party predicts that the planned number of all-day meals over a period of 36 months will be: a patient with a referral – 639,000 meals, a commercial patient – 51,000 meals. The above amount is an estimate.

a. The term "patient nutrition" should be understood as the contractor's own preparation (cooking) and delivery of ready meals to individual buildings of the Ordering Party, in accordance with the orders placed by the Ordering Party and at the times specified by the Ordering Party.

b. Preparation of meals will take place at the catering point organized in the "Zimowit" building. The catering point consists of a number of rooms, which consist of a proper kitchen, kitchen facilities and storage and cleaning rooms.

c. Serving meals to patients undergoing treatment at the Ordering Party will take place in the dining room, organized in the buildings "Zimowit", "Eskulap", "Maria", "Teresa", "Polonia", "Świtezianka", "Gołąbek" and "Anna". The individual dining rooms include rooms, which consist of: waiter switchboards, washrooms and cleaning rooms.

d. The rooms referred to above (the feeding point organized in the "Zimowit" building, dining rooms, organized in the buildings "Zimowit", "Eskulap", "Maria", "Teresa" "Polonia", "Świtezianka", "Gołąbek" and "Anna") will be transferred to the Contractor partly as part of the lease and free use, for the duration of the feeding contract.

e. The rooms referred to above have been partially equipped by the Ordering Party with kitchen equipment and equipment. This equipment will be handed over to the Contractor as part of the loan, for the duration of the feeding contract.

f. The ordering party predicts that the planned number of all-day meals over a period of 36 months will be: a patient with a referral – 639,000 meals, a commercial patient – 51,000 meals. The above amount is an estimate and may be reduced depending on the needs of the Ordering Party.

g. The Contractor will be obliged, within 30 days from the date of signing the contract, to develop and implement the HACCP system in all the Ordering Party's facilities in which the nutrition service will be performed.
