1. The subject of the contract is the purchase of a medicinal product - botulinum toxin type A - used in the treatment of focal spasticity in cerebral palsy and delivery to the Department of Hospital Pharmacy in Józefów, 3 Maja 127 Street (05-420 Józefów).
2. The order is divided into lots.
3. The Contracting Authority shall allow partial tenders to be submitted in the following lots:
Part 1 - Botulinum toxin type A BOTOX- 100j. form: ampoule in the amount: 2200 pcs.
Part 2 - Botulinum toxin type A DYSPORT – 500j. Form: ampoule in the amount: 400 pcs.
Part 3 - Botulinum toxin type A DYSPORT – 300j. form: ampoule in the amount: 300 pcs.
Botulinum toxin type A BOTOX – 100j.Purchase and delivery of botulinum toxin type A BOTOX – 100j. form: ampoule in the amount of: 2200 pcs.
