1. The subject of the contract is the successive delivery of laboratory materials for organizational units of the Medical University of Gdańsk.
2. The Ordering Party has divided the subject of the contract into 5 packages (parts):
Package 1 – Package 5: laboratory materials
The assortment of the subject of the order has been described in the material and price form constituting Annex 3 to the SWZ.
3. The offer (Annex 1 to the SWZ) should be accompanied by:
a) ESD statements,
b) declarations on not being subject to exclusion from the proceedings referred to in Chapter V of the SWZ – Annex 5 to the SWZ
c) statement - Annex 5.1 to the SWZ
d) completed material and price form – Annex 3 to the SWZ
e) if DOT – power of attorney to submit the offer,
f) if dot – a document confirming the appointment by the Contractors jointly applying for the contract of a representative to represent
Detailed provisions regarding the required documents, the form of their submission and signing, and the conditions for implementation are included in the SWZ.
