1. The subject of the contract is the purchase of medical equipment for Copernicus PL Sp. z o.o. (mobile lamps, portable ultrasound machine, vascular laser, incubator, surgical diathermy). The order consists of 5 parts.
2. A detailed description, assortment and quantities are included in Appendix 1B to the Terms of Reference – Description of the Subject of the Contract (OPZ).
3. The grounds for exclusion of Contractors from the proceedings are indicated:
- in Article 108(1) of the PPL, which occurred during the relevant period specified in Article 111 of the PPL,
- in Article 7(1) of the Act of 13 April 2022 on special solutions for counteracting support for aggression against Ukraine and serving to protect national security,
- in Article 5k(1) of Council Regulation (EU) 2022/576 of 8 April 2022 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine.
Part 1 - Mobile lamps1. The subject of the contract is the purchase of medical equipment - mobile lamps for the Copernicus PL Hospital Sp. z o.o.
2. A detailed description, assortment and quantities are included in Appendix 1B to the Terms of Reference – Description of the Subject of the Contract (hereinafter referred to as the Terms of Reference). The Contracting Authority attaches this attachment to the offer. By completing the items indicated in this Appendix, the Contractor confirms that the requirements specified by the Contracting Authority are met.
3. The method of execution of the subject of the contract is specified in the draft agreement - Appendix No. 3 to the Terms of Reference, one-time delivery.
3.The offered subject of the contract must be complete, of a high standard both in terms of quality and functionality, as well as free from material, construction and legal defects.
4. The offered subject of the order must be admitted to trading and use in accordance with applicable law.
5.The subject of the contract must comply with the Act of 7 April 2022 on medical devices (i.e. Journal of Laws 2022.974 of 2022.05.09.), if the Ordering Party requires offering an assortment constituting a medical device.
6.The Contracting Authority shall not reserve the possibility of applying for a contract only by the Contractors referred to in Article 94 of the Pzp Act.

Part 2 - Portable ultrasound machine.1. The subject of the contract is the purchase of medical equipment - a portable ultrasound machine for the Copernicus PL Hospital Sp. z o.o.

The Contracting Authority attaches this attachment to the offer.
