The subject of the contract is group life insurance for employees of the Supreme Chamber of Control and members of their families, covering the events referred to in point II.2.4).

1. The subject of the contract is group life insurance for employees of the Supreme Audit Office and their family members, covering the following events:

1.1 Death of the Insured.

1.2 Death of the insured as a result of an accident, accident at work, accident at traffic and traffic accident at work.

1.3 Death of the Insured as a result of a heart attack or stroke.

1.4 The occurrence of permanent damage to health of the Insured as a result of an accident, heart attack or stroke.

1.5 The occurrence of permanent incapacity for work and independent existence of the Insured as a result of an accident or illness.

1.6 Serious illness of the insured person.

1.7 Passage of a surgical operation by the Insured.

1.8 Stay of the insured person in hospital as a result of illness, accident / traffic accident / accident at work / traffic accident at work, heart attack or stroke.

1.9 Death of the spouse or life partner of the insured.

1.10 Death of the spouse or life partner of the insured as a result of an accident, accident.

1.11 Death of a child.

1.12 Death of a child as a result of an accident.

1.13 Orphaning of a child.

1.14 Death of the parents of the insured person or parents of the spouse or life partner of the insured.

1.15 Death of the parents of the insured person or parents of the spouse or life partner of the insured as a result of an accident.

1.16 Childbirth.

1.17 Stillbirth.

1.18 Lump sum for medicines.

1.19 Conducting specialist treatment with the insured.

1.20 Lump sum for ICU/ICU stay.

1.21 Lifetime Continuation Clause with Guarantee of Conditions.

1.22 Serious illness of a child insured in Scope IV.

1.23 Hospital stay of a child insured in Scope IV.

2. A detailed description of the subject of the contract includes:

1) Annex 1 to the SWZ – Description of the subject of the contract (OPZ) together with attachments,

2) Appendix No. 2 to the SWZ – Model Agreement (the Contractor is obliged to perform the contract on the terms and conditions described in the Model Agreement),

3) Annex 3 to the Terms of Reference – Model Offer Form (the document contains the requirements of the Contracting Authority regarding the scope of the subject of the contract).
