1. The subject of the contract is the delivery and installation of equipment for the internal medicine department as well as medical and disinfection equipment to counteract COVID-19, as part of the project "Expansion, reconstruction and retrofitting of Kociewski Centrum Zdrowia Sp. z o.o. in Starogard Gdański and improving the qualifications of medical staff in order to increase the availability of specialized health services in the field of civilization diseases" implemented under the Regional Operational Program of the Voivodeship Pomorskie 2014-2020, Priority Axis 7 Health, Sub-measure 7.1.2 Health care resources.

2. Delivery of equipment to the place indicated by the Ordering Party shall take place by an appropriate means of transport, at the expense and risk of the Contractor.

3. The contractor shall be responsible for the proper preparation of the equipment for operation, commissioning and training of personnel.

4. The description of the subject of the contract (OPZ) constitutes Appendix No. 1 to the SWZ.

1. The subject of the order is the delivery and installation of equipment for the internal medicine department as well as medical and disinfection equipment to counteract COVID-19.

2. The description of the subject matter of the order (OPZ) is Annex 1 to the SWZ.

3. The Contractor shall grant at least a 24-month warranty period.

4. The Contractor is obliged to execute the contract on the terms and conditions described in the OPZ constituting Appendix No. 1 to the SWZ and in the draft provisions of the contract constituting Appendix No. 9 to the SWZ.
