Jolanta Paszkowska, MSc, Eng., constituting an attachment to this application in accordance with the Project Co-financing Agreement under Priority Axis 11 Revitalization of the regional space of the Regional Operational Program of the Malopolska Region for 2014-2020, Agreement No. RPMP.11.04.00-12-0111/18-00-XVII/117/FE/20 of 19.01.2021. to the extent described in the Application Form, item C.2.3 Tasks and activities implemented in the Project, Measure 1.2.

The material scope of the subject of the contract includes in particular:
a) the service of making custom-made furniture along with their transport and assembly,
b) delivery and assembly of built-in household appliances,

e) providing as-built documentation, i.e. a quantitative acceptance protocol specifying: the subject of delivery, a quantitative and valuable list (gross amount) of individual elements of equipment, warranty cards, certificates, attestations, declarations of conformity and instructions for the use of equipment.

Note: Delivery of equipment and equipment will be possible not earlier than after the completion of construction works in the building being the place of performance of the subject of the contract.
