The subject of this contract is a group life insurance service for employees of the Sergeant Grzegorz TCrew Independent Public Health Care Institution of the Ministry of the Interior and Administration in Katowice and their family members. The description of the subject of the contract together with clauses extending the scope of insurance cover is set out in Annex 1 to the SWZ. As a result of the tender procedure, a contract will be concluded in accordance with Annex 2 to the SWZ. The Contractor shall issue policies specifying the subject and scope of liability, sums insured, amount of premiums, and shall also provide the General Terms and Conditions of Insurance applicable to matters not regulated in the Terms of Contract.
