Task No. 1 - Modernization of the Rehabilitation Center

1. The Contracting Authority informs that before submitting the bid, it requires a site inspection at the place of performance of the contract in accordance with Article 131(2)(1) and (2) of the PPL, consisting in an inspection of the place of performance of the subject of the contract, i.e. the surroundings and the building, or checking the documents regarding the contract that are at the disposal of the Contracting Authority and which will be made available to entities submitting their willingness to participate in the procedure.

2. In order to arrange a site visit or get acquainted with the documentation located on site at the Ordering Party, please contact the persons designated to communicate with the Contractors.

3. The Contracting Authority states that the date of the on-site inspection will be possible on the designated days, i.e.: 24.07.2023, 31.07.2023, 03.08.2023. - at 11:00 o'clock

Meeting point: St. Padre Pio Provincial Hospital, Monte Cassino 18, 37- 700 Przemyśl. Attendance is mandatory.

4. Confirmation of the on-site inspection will be a Statement signed by persons representing the Contractor and the Contracting Authority. The person representing the Contractor must be authorized to perform this action on the basis of registration documents or power of attorney submitted in the original or a copy certified as being in conformity with the original. The template of the Statement on conducting the on-site inspection is attached as Annex 10 to the SWZ. The statement shall be drawn up in 2 identical copies, one for the Contractor and one for the Contracting Entity.

5. The Contractor's costs related to participation in the on-site inspection shall be borne by the Contractor.

6. If the bid is submitted in the procedure without a site inspection at the Contracting Authority or without checking the documents necessary for the performance of the contract available on the spot at the Ordering Party, it shall be rejected pursuant to Article 226(1)(18) of the PPL.
