Description:

North Staffordshire CCG are seeking to engage providers who are able to offer provision of community based Falls Responder Service within North Staffordshire.

The key objective is to appoint one provider to deliver a Response Service for people at risk of falling and injuring themselves in their own home. The service will provide safety for people who are vulnerable due to their risk of falling and not being able to get up again. It will increase independence through provision of a safer home environment and it will reduce the number of unnecessary 999 calls and subsequent conveyances to A&E for people have fallen but are uninjured beyond needing first aid and/or reassurance at home. The service will prevent further complications and resulting hospital admissions which occur when people are left unattended for long periods of time.

The service will :-

— Help the patient up after a fall, using the latest lifting cushions.

— Provide first aid to deal with a wide range of injuries.

— Provide clinical triage to ensure the patient is safe to stay at home

— Provide a welfare visit to see how the patient is after a fall or at any time if they feel unwell on request.

— Check they are ok if they activate the community alarm but the Control Centre cannot get in touch.

— Communicate with other agencies (if the patient agrees) to help resolve any issues.

— Be available for as long as the patient requires it. An exit strategy will be agreed for patients who regain their pre-fall health and no longer require the service.

Given the geographical footprint of North Staffordshire, it is anticipated that the provider will ensure that the service supports a fast response service both in the north and south of the catchment area.

Any appointed provider will be required to work alongside our existing primary care service.
